Independent Haulers
We utilize the services of other trucking companies on many of our own projects and external jobs. If you would like to become one of our independent haulers, please view the information below.
To start the process on becoming one of our independent haulers, we need the following information / forms completed and returned to the Main Office.
- Independent Hauler Agreement
- Updated Agreement coming soon! - W-9
- Certificate of Insurance listing Tezak Heavy Equipment Co., Inc. as additional insured. (See Attached Example )
- Exhibit A Forms (One for each driver, signed by both the driver and yourself).
Please note that the signatures on Exhibit A allow us to check MVR’s on your drivers. These are to be updated every time a new driver is hired or one is terminated. A new driver will be unable to drive until we have a complete form and Tezak has a chance to run a MVR or you provide us with a copy of their MVR. Please print extra forms at your convenience.
In addition to the above, the following information is also required:
- Drug testing verification letter , which should include an agreement with a drug testing company.
- A copy of your drug policy.
- A copy of your safety policy.
- Letter of DOT Compliance .
- Phone number, fax number and cell number.
- A copy of your registration for all trucks that will be hauling out of our pit. This should match the list on the contract.
- A copy of each driver's medical card and drivers license.
- Workers' Compensation Exemption Certificate , only for Owner/Operators that don't have any employees.
Until the above information is received, you will not be considered an Independent Hauler for Tezak Heavy Equipment Co., Inc.
This agreement and attachments are only for vehicles owned by you. If you have any trucks leased to you, you need to contact the office. They will need to fill out the same paperwork as you with the same requirements.
